Complete a Passport Application
You can complete it online through the State Department's website, or print and complete it by hand.  New applicants, renewals, name changes or corrections, and lost or stolen passports each require a different application. 

Applicants must provide 2 types of identification, with copies of each:
Proper identification (valid driver's license)
Proof of U.S. citizenship (certified birth certificate)

Passport Photo:
For an additional fee, this post office will take your photo for you.
You may also provide your own 2" x 2" color photo with a full face, front view (head should measure between 1" and 1 3/8", and a plain white or off-white background, taken within the past 6 months

Passport Fee (including a $25 Execution Fee):
$135 for applicants 16 years of age and up
$105 for applicants under age 16
$  15 for passport photos taken on site
